Barpartite Medial Cuneiform with probabletear of medial itertarsal ligament. Read this was rare? HELP! :-) ?

I continue to have mid foot pain, swelling and aching of my right foot. Was given an MRI and finding shows: Barpartite Medial Cuneiform with edema and probable tear of medial itertarsal ligament alongside medical cuneiform.

I think I was misdiagnosed last year Nov 2012 with a fracture of medial cuneiform. I was put in a boot for 8 weeks and then MRI'd and was told it was a "Non-Union" Fracture of Medical Cuneiform, given a bone stimulator and told surgery was only option to fix a non-union FX or I could try orthotics and "live with it". Since it was Christmas - I chose to postpone surgery and live with it.

However, I recently began having problems with the foot again. If I exercise at all or am on the foot for long periods, it will swell up over the medial cuneiform and ache. Sometimes ache so bad I want to cry. Which prompted me to see a DPM thinking I "re-fractured" it. But I was given this report by the Imaging Center.

I read this is rare (3 reported cases) and that surgery corrected the problem in two and one rugby player had CT guided injection. However, I read one DPM site where and MD said he had seen and treated 4 himself. just never published.

So my question is what now??? I like walking...It's kinda important and I enjoy exercise, it is vital to health - so what do I do? It sounds like my MD is not very aware of what to do? I would like to go in with some knowledge to help discuss treatment options.

Tricky one. Its is very rare. I have not seen one!
I think the first step if to just clarify if it really is a fracture or really is bipartite.

If it is as bad as what you say, I would be surprised if orthotics would help. Surgical repair to the "fracture" or "unite the bipartite" would be an option - and even investigate the status of the possible ligament tear at the same time.

Sorry, I really can't offer any more.
