For 6 months now I have one single toe that itches severely on top, bilateral. after it itches it feels a little bit painful like needles in that area. It is not athletes foot. And no rash. When I itch the toe it turns red and hot.

That needs investigation further. It does tend to suggest a neurovascular problem.
