2 months ago diagnosed with probable sesamoid fracture. Custom orthotic and hard shoe led to only mild pain reduction. Then, after a dancing at a reunion with a sesamoid pad in flat shoe, developed worsening pain, redness, whole foot swelling.Presumed gout yet joint aspirate negative for urate and calcium.
Cast placed a week later. Recent CT shows (besides known hallux valgus) bony/cartilaginous fragments in joint. Any thoughts on how to proceed since I presume these will never resorb and were the cause of the horridly painful "pseudo gout" episode??

They probably need to be surgical fixed.
