I am a 58 yr old female, diabetic and overweight. For the past month I have had severe pain/tender to the touch on the top tip of my 4th toe on my left foot. I haven’t stubbed or broken it. No foot surgeries and blood sugars are under control. No signs of neuropathy as of yet.

Based on the information you have provided, no idea why.
Given that you have diabetes, it could be early stages of neuropathy; or it could be a chilblain; or it could be a corn; or it could be any one of 100's of different things.
