I am a Dermatologist and work in New York City.
I dermatologist and need to ask your advice.
I am pretty desperate.
I have seen 3 Podiatrists in NYC and they all have different modes of action as to how to handle my foot ulcer.
Well, 8 weeks ago I had a callus on my Left 5th toe MCP joint. I debrided the callus which turned into a 10mm x 10mm ulcer which slowly healed with antibiotics, saline washes and Mupirocin ointment twice a day.
So, now I have developed a non healing chronic ulcer with undermining, edema, and pain. Slight drainage which has been cultured twice and NO growth, no MRSA. But there is slight non bloody drainage that has no fowl smell to it.
I am a healthy 49 year old male without Diabetes or any other pertinent medical history. I am an athlete and am on my feet all day at work.
I am going for an MRI with/without contrast on Tuesday to r/o Osteomyelitis and/or an abscess.
Is surgery the way to go to clean out the undermined tissue? and close up the wound?
Do you have any thoughts??

Given your age and health status this should not be happening. It most likely means that surgery to clean out a possible problem is warranted. It hard to say without definitive imaging.
