Chronic Pain - Posterior Side of Both Legs/Ankles - Volleyball Player - Please Help! ?

Hello, I am a competitive volleyball player. Almost a year ago I developed what I thought were shin splints in both legs after playing indoors. Well now, almost one year later, I still have a chronic throbbing/burning, even sharp, type pain at times in both legs in the post tib tendon and down into the ankle area and below the ankle. I feel this pain with no activity. It will just feel like a deep ache, or, when it's bad, it will burn. It can be significant at times, and it has really been a nagging discomfort and certainly interfering with my daily life activities. I have had MRI's and X-Rays done. Nothing more than inflammation and tenosynivitis has shown up. I have had bloodwork, an EMG done, deep tissue work, seen a vascular doc, a chiropractor, and several foot docs and orthopedics - none of that has worked. I have even gotten custom orthotics which has provided only little relief. If I press into the post tib tendon area it feels tender to the touch. I have also recently developed what feels like bumps or lumps (knots?) beneath the skin that I can clearly feel if I run my hand along the course of the tendon from my ankle up to my knee. Does anyone have any speculation as to what this can be or what I can do? I have been doing a lot of mobility and stability strengthening and stretching exercises already.

A few things I am speculating this could be: PTTD, posterior tibial tendinitis, tendinosis, tarsal tunnel syndrome, compartment syndrome. (However, even if it is one of those, I have not been able to find the solution to get it to go away).

My beach v-ball season returns at the end of this May so I have 4 months to get this better. I really need to find a solution because I was already out almost half the season last year with this problem. Thank you everyone for the assistance.

It does sound like post tib tendonitis and the standards treatments that are used for that appears to have been done, so not sure I can offer any more.

The "knots" may be trigger points in the post tib muscle, so that may be worth pursing.

Apart from that all I can suggest is more rest to let it settle and then a careful progressive loading program for the tendon as its rehabbed.
